Employer Paid Short Term and Long Term Disability
(Salaried Managers and Full-Time Corporate Office Staff Only)

• Short Term Disability insurance is for when the employee is disabled outside of work and cannot perform their job duties. This also includes pregnancy.
  
• You receive 66 2/3% of your wages, up to $800 per week.
  
• Benefits begin on the 15th day.
• If hospital confined, you will be paid from the day you are admitted.
• You will receive payment up to 24 weeks, depending on disability.
• Checks are sent to you weekly.
• This is a company paid benefit.

• This insurance is for a long-term illness and after the employee has been on short term the maximum of 24 weeks.
  
• You will receive 60% of your wages, up to $4,000 per month.
• You may collect until age 65.
• Checks are sent to you monthly.
• This is a company paid benefit.
