Fashion Focus by Davis Vision - to find a provider:

*Go to http://www.highmarkbcbs.com
*In the blue "Find Insurance" box, click on "Groups & Companies"
*Scroll down to the bottom of the page to Vision Benefit and click the "Go" box
*At the bottom of the page, click the link to find a network provider
*Click OK when prompted that you are leaving highmarkbcbs.com
*Enter zip code and radius for a list of providers

Single:  $5.18 per month ($2.59 per pay)
Family: $15.02  per month ($7.51 per pay)

Once enrolled in Hoss's Vision Insurance Plan, you must remain in the plan for a period of one year.
